The Governing Council has authorised the gradual increase of on-call pay, night work, and holiday pay for over 60,000 SERMAS professionals. The measure, agreed with the unions on June 26, will take effect from August 2026.
More than 60,000 professionals from the Madrid Health Service (SERMAS) will see their pay for on-call duties, night work, and holidays increase starting next August. This was authorised on Wednesday by the Governing Council of the Community of Madrid, following an agreement reached on June 26 between the General Directorate of Human Resources and Labour Relations of SERMAS and the trade unions at the Health Sector Table.
The increase affects both healthcare and non-healthcare staff working in the region's public hospitals, primary care centres — including the Continuous Care Points (PAC) — and the Madrid Medical Emergency Service (SUMMA 112).
In the case of doctors, the pay for an on-call shift on a working day will rise to €484.31 from August 2026, increasing to €513.36 from 2027. For on-call shifts on holidays, the compensation will be €748.57 this year, reaching €793.48 from January 1, 2027.
Another significant change is that, starting next year, Saturday will be considered a special day for pay purposes, which will mean an additional bonus for professionals working that day.
The agreement also includes improvements in vacation, leave, licenses, and work-life balance. A new consolidated text will be developed to replace the current regulations from 2021 and update the applicable regime for SERMAS workers.
These measures add to others already implemented, such as the financial compensation for the night of January 5 to 6 (Three Kings' Night) and the extension of the continuity assistance supplement.
With this package, the regional government aims to improve the working conditions of Madrid's healthcare professionals, in a context of union demands for wage parity and the reduction of temporary contracts in the sector.
The unions have positively assessed the agreement, although they warn that there are still pending issues, such as the regulation of working hours and the reduction of waiting lists. For its part, the Ministry of Health is confident that the measure will help retain talent and make the Madrid public health system more attractive.
The gradual increase will be implemented in two phases: the first from August 2026, and the second from January 2027. SERMAS professionals will be able to check the new amounts on their payslips starting in August.
